If a published provider fact has changed or conflicts with its source, a human reviewer should inspect the record.
Demonstration route
This shell does not collect a message or contact details. In a deployed workflow, corrections will be routed through a protected editorial process without asking for health information.
What happens next
- The submitted fact and supporting source enter an editorial review queue.
- The current public record remains explicit about its status while evidence is checked.
- A reviewer records the decision, source, date, and any unresolved disagreement.
Provider-submitted information is not automatically treated as independently verified. Public corrections do not require health information.
